  1. Passport / British Birth Certificate / Eligibility to Work (Letter from the Home Office)
  2. Proof of Addresses (Utility Bill, Bank Statement, Council Tax Letter, HMRC Letter, Tenancy Agreement Letter from Local Authorities etc. with your Name and Current Address) (date must be within 3 months)
  3. Passport size photographs or a photo taken in the office for your ID card
  4. National Insurance Number Card or Proof of National Insurance Number
  5. Two professional referees – one must be the last or present employer
  6. Disclosure (issued by the Disclosure Barring Services) by LifeCome Care Ltd
  7. Driving Licence (if applying for driving work)
  8. Copies of Relevant Certificates and Curriculum Vitae
